Doctors Approach Financial Policy January 1, 2014
Our objective is to provide patients with the highest quality healthcare in most cost-effective manner. However, the ability of Doctor’s Approach to achieve this objective depends greatly on your understanding of our financial policy. It is more important than ever that you understand your insurance coverage and financial obligations during this period of transition.
We ask all patients to pre-pay for their office visit and any procedures. We will refund any amounts immediately upon receipt of reimbursement from your insurance company. If you have medical insurance, we will file the insurance claims on your behalf. Even though we file insurance claims for you, we need your active participation in the insurance claims process.
If you have negotiated a new group health plan or individual health plan you will need to verify that Doctors Approach is a contracted provider. You can contact your insurer or Doctor’s Approach through our website, www.DaDerm.com, for a list of insurers or by telephone: 517.993.5900.Doctors Approach accepts cash, checks, and credit/debit payment cards. We also offer Care Credit.
Medicare Patients
As a participating provider of Medicare Part B (physician services), Doctor’s Approach will bill you only for your Medicare coinsurance, deductible, and any services rendered but not covered by Medicare. All other services will be billed directly to Medicare.
Note: We will inform you of services not covered by Medicare Part B before we provide them. You will authorize us to provide these services with your signature on the appropriate Medicare waiver form and accept the financial responsibility for them. If you have Medicare Part A only, the services you receive from our practice will not be covered by Medicare.
Commercial Insurance Patients
(Including Self-Funded Plans and Individual Plans)
Remember that your insurance contract is between you and your insurer. Because of increased contract complexity that has been intensified by the Affordable Care Act, financial obligations among patients will vary dramatically and we ask your assistance.
- We ask that you to know your co-pay amount and pay it at the time of visit.
- We ask that you understand your deductible, coinsurance and coverage whether it is a group or individual plan
In all cases, our staff will give you an estimate of your financial obligations prior to a procedure be scheduled. We ask you to pre-pay any estimated amounts that will not be covered by the insurer. Subsequently, if you require or want additional a non-covered procedure(s) we will provide you an estimate of your obligation before we proceed.
We will file your insurance claim for you so that you will receive credit for your payments toward the satisfaction of deductible and/or coinsurance. If your insurance company rejects your claim, you are financially responsible for the balance. We ask you to pay it upon receipt of your statement.
HMO/PPO/Managed Care Plans
HMO/PPO/managed care plans may require that you obtain a referral in order to receive care from a specialist. It is your responsibility to obtain this referral, if required. Please inform the registrar when you are scheduling your visit and bring any referral forms with you. You will be asked to pay for the unauthorized services. Please insure that you have the approval of your referring physician for payers that require it.
Patients with No Insurance
Generally, if you have no insurance we ask you to pay in full at the time of your visit. If you need additional services, including but not limited to medications or injections and medical procedures, we will provide you an estimate prior to performing those procedures. We ask that you prepay for those services.
