As a new patient, please bring your actual insurance card and picture identification. Any co-pays or deductibles will be collected at the time of service. If you are unaware of your co-pay amount, a standard $40 is collected when you check in.
We participate with many insurance plans, click here to see if we participate with your health insurance, feel free to call our office at 517-993-5900 with any questions regarding your insurance, you may also need to check with your insurance company prior to your visit to verify if authorizations are required.
Please bring all information regarding your condition including any of the following to your visit:
- Pathology Results
- Laboratory Test Results
- List of oral and topical medications used for your condition
These results and medications are extremely beneficial in our evaluation of your dermatologic problem(s).

You can make an appointment by calling the office at (517) 993-5900 . If you are an existing patient, you can schedule your visit through the patient portal.
Cancel Appointment/Rescheduling Policy
If you must cancel an appointment, please call the office at (517) 993-5900 . The staff will work to reschedule you as soon as possible. Good communication is the key to serving you better. If you need to change or reschedule appointments, kindly give us at least 48 hours and 72 hours notice for surgery patients. If you fail to reschedule your appointment without appropriate notice or fail to show for your visit, a fee of $75 will be charged to your account

- A thorough full-body exam involves checking the skin and moles of concern for signs of skin cancer or any abnormalities. Our extensive exam utilizes special lighting, followed by additional diagnostic tests if needed.
